This Service Agreement ("Agreement") outlines the terms under which Uphold Institute ("Service Provider") will provide sports activities and related services.
Service Provider: Uphold Institute - Sports Ministry Team
Client: Parent or guardian signing up their child.
Specific Activities: Various sports activities: Football, Hockey, Frisbee, Netball, Volleyball, Community games and other related non-contact games.
Schedule & Duration:
During School: Every Saturday from 8:00 AM - 0:00 AM, starting July 26th, 2025.
During Holiday: Every Wednesday and Saturday from 8:00 AM - 10:00 AM,
Location: Kavumba Recreation Center.
Inclusions: Basic first aid.
Exclusions: All medical, transportation, food and drinks are the responsibility of the parent or guardian. Children must always carry their drinking water.
Access Fees:
Children 3-12 years: 210,000 Ush per child per year, non- refundable, paid to Uphold Institute. This can be paid at once or in installments.
This excludes direct requirements: jersey, shoes, etc.
The cost for jerseys will be communicated and collected in due time, in order to produce a uniform branded jersey.
Payment Methods: MTN MOMO Account: 757528. No cash payments.
Pay to access system: Children must appear only if payment has been made. Late payment means borrowing from the institute and we do not have a lending policy related to our services.
Refund Policy: No refunds for missed sessions. Pro-rata refund is applicable if the program is cancelled by Uphold Institute.
Late pick-up charge: 10,000 Ush per hour.
Attendance: Punctuality and full attention in attendance is essential.
Medical Information: Parents disclose relevant medical conditions that can affect a child during exercises.
Emergency Contacts: Provision of up-to-date emergency contact information.
Code of Conduct: Adherence to the Institute's rules and codes of conduct for participants and parents/guardians during activities.
Equipment: Any child MUST carry their own drinking water.
Hygiene: Adhere to minimum hygiene standards - No sharing fluids using the same bottle, keeping the environment clean, and related others.
Dropping and Pickup of their child including sign-in and sign-out.
Safeguarding Policy: Please read Uphold Institute's Child Safeguarding Policy.
Emergency Procedures: In case of emergency the parent will be contacted first for a consent on an appropriate action.
Medical Consent: Should a parent be unreachable during an emergency, Uphold Institute will prioritize the child's immediate safety and inform the police without delay. The parent will remain responsible for all incurred costs.
Limitation of Liability: While provided the Institute exercises reasonable care, participation is at the client's own risk.
Indemnification: As a parent or guardian, I acknowledge and agree that participation in Uphold Institute's sports activities involves inherent risks, and I voluntarily assume all risks of injury or damage to my child and property. I hereby release, indemnify, and hold harmless Uphold Institute, its staff, coaches, and volunteers from any and all liability for any injury, loss, or damage that may occur during participation, unless caused by the gross negligence or willful misconduct of the Institute.
By Client:
The Client may withdraw their child from the sports program in the event of a documented medical condition or injury preventing the child's continuation in sports programs.
By Service Provider: The Uphold Institute reserves the right to terminate this agreement due to:
Non-payment
Breach of the code of conduct
Safety concerns
This Service Agreement is governed by the laws of Uganda.
